Starting with the 2015-2016 school year, all medical providers conducting school sports physicals must have completed a professional development module now required by the New Jersey Department of Education. Our providers will attest to this certification on the Pre-Participation Physical Examination (PPE) form, as well as provide a copy of their certificate to aid your school in compliance with this new State law.

The School Sports Physical Exam involves a medical history and a physical exam. The goal of these exams are to evaluate an athlete’s general health, fitness level, risk of injury and physical well-being. You can prepare for the exam by downloading the PPE form at the following link, and completing pages 1 and 2.
English (PDF) or Spanish (PDF)

If by chance you don’t have the PPE form before your exam, Access Medical Associates has both the English and Spanish versions of the form available.
